Family Health Team Services

- You must complete your meet-and-greet appointment (first appointment) before using online booking.

Services are available to:
- Patients attached to an FHT NP or Affiliated Physician,
or
- Ontarians without a physician or with a physician outside our team who live in: Admaston/Bromley, Burnstown, Calabogie, Dacre, Douglas, Greater Madawaska, Griffith, Haley Station, Horton, or Renfrew.
Interprofessional Health Providers - Mental Health Counsellors (Casey Mosco, Kristie Goulet, Sydney Friske), Kinesiologist (John Buffam), Registered Dietitian (Morag Townsend) (book an appointment)
- Online booking is for follow-up appointments only.
- Please do not use online booking if you have not yet completed an initial intake or assessment.

Unattached Cervical Cancer Screening (book an appointment)
This service is available to Ontarians in our region who do not have a family doctor or nurse practitioner. If you already have a physician or nurse practitioner, please book your screening with them.
You are eligible to book if all of the following apply:
- Have a cervix
- Have had any type of sexual activity
- Are between the ages of 25–70
- It has been 3 years or longer since your last Pap test
- Do not have a family doctor or nurse practitioner
